Palliative Care Improves the Quality of Life for Geriatric Patients
Geriatric Patients and their family members facing problems, associated with chronic, serious progressive and life limiting disease require, PALIATIVE CARE to improve their quality of life.
This is done by:-
- Prevention and relief of suffering of patients.
- Early identification and impeccable assessment of the patrient to know his exact health status.
- Charting out immediate care plan to relieve acute distressing physical symptoms, pain, breathlessness and other distressing symptoms.
- Making a long term care plan, after finding out the emotional, psychosocial, spiritual problems of the patients and their cultural and religious beliefs and taking their consent.
- Involving members of the multidisciplanary team of Pain & Palliative Care Specialist/ Superspecialist, Psychologists, Rehabilitation Specialists, Physiotherapist, Social worker and Dietician as per the needs of the patients.
- Implementing the care plan as per patient’s needs in OPD, Daycare, Wards, Rehabilitation (Outdoor and Indoor).
- Preparing the family on the following:-
- Expected future problems and their prevention.
- Managing patients at home, after preparing the desired infrastructure at home.
- Training of relatives/ care givers/ attendant on how to manage the patient at home.
- Bringing them for regular follow up
- Reporting to the emergency in case of any serious/ distressing symptoms
